Medication Therapy Management, MTM



Medication Therapy Management (MTM) encompasses a broad range of health care services provided by pharmacists.  The pharmacy profession defines MTM as a distinct service or group of services that optimizes therapeutic outcomes for individual patients. Medication Therapy Management (MTM) is a patient  centered process of care that includes assessment and evaluation of the patient and his or her complete medication therapy regimen rather than focusing on an individual medication product.   MTM services can be performed face to face or over the phone.  interactions in person help establish the pharmacist-patient relationship while also allowing the pharmacist to visually asses the patient for sings and symptoms of possible medication related problems such as confusion, lethargy,  bruising,  muscle spasms and tremors.   During the assessment, the pharmacist may also conduct physical assessments, vital sign measurement, as well as testing blood sugar.


The History of Medication Therapy Management (MTM).

Pharmacist have provided individual patient care services in numerous setting for  decades.  In the 1990’s, these services were often referred to as pharmaceutical care.  The term Medication Therapy Management has been in wide use for almost twenty years since the Medication Prescription Drug, Improvement, and Modernization Act of 2003 (MNA)  put MTM on the health care map as part of the Medicare Part D befits.  And owing to this, MTM services have expanded and are projected to continue to expand.
